Negative Pressure Pulmonary Edema after Appendectomy - A Case Report -

Abstract
Postoperative pulmonary edema is an uncommon, but potentially life-threatening complication. The causes of postoperative pulmonary edema are preexisting cardiac disease with fluid overload, postoperative hyponatremic encephalopathy, head trauma, pheochromocytoma and negative intrapleural pressure developed by upper airway obstruction. We experienced a case that pulmonary edema occurred in the recovery room after appendectomy in
otherwise healthy male patient except for dehydration before surgery. He had to go to intensive care unit for treatment of pulmonary edema, but recovered rapidly and discharged uneventfully.
The causes of postoperative pumonary edema are overloading of fluid in patients with preexisting cardiopulmonary dysfunction1), central nervous system injury2), acute respiratory failure3), and negative intrapleural pressure caused by upper airway obstruction4). Negative pressure pulmonary edema (NPPE) occurrs unpredictably even in otherwise healthy
patients undergoing relatively simple operation and often needs endotracheal intubation and admission to intensive care unit due to hypoxemia and respiratory failure. We report a case that 41-year old, healthy male patient undergoing appendectomy experienced acute pulmonary edema in the recovery room.
